Our Healthcare Team

The interdisciplinary Prime One Hospice Inc. team is made up of trained caregivers who put individual patient and family needs first:

NURSES are experienced in assessing and managing pain; they provide hands-on care.

SOCIAL WORKERS are friendly and skilled listeners who can offer emotional support and help with financial issues and planning.

PHYSICIANS collaborate with the patient’s primary physician and are available for pain and symptom management consultation, as well as to make house calls as needed.

HOME HEALTH AIDES help with personal care, such as bathing and mouth care; they also can help with light cleaning, light laundry and occasional shopping.

CHAPLAINS offer spiritual support.

VOLUNTEERS, specially trained in hospice, are important elements of the team who are available when they are needed most.

Any patient who enters hospice must be evaluated at the time of admission to the hospice program, and if there is a dietary need, a dietitian should be provided to assist and educate the patient and family. Common problems such as nausea and vomiting are controlled with medications in some cases, while for others, a change of diet is sufficient. Hospice patients and families need to be aware of the changes occurring in the patent's body. Each disease process has different effects on the patient.

OCCUPATIONAL THERAPY/PHYSICAL THERAPY: An Occupational Therapist/Physical Therapist can evaluate the ability of your loved one to accomplish his activities of daily living to do the normal everyday tasks that are commonly performed, such as bathing, getting dressed, brushing his teeth, and feeding himself, walking, in and out of bed, transfer from chair to bed, into the bathroom, to and from a car or wheelchair, etc. If there are difficulties in performing these types of tasks, the therapist can devise alternative ways of accomplishing these tasks which allow the patient to remain more independent (and less dependent upon others) which can often help tremendously with the patient's self-esteem and attitude.

Sometimes different adaptive or assisting devices are used to help patients accomplish these tasks. The therapist can explain how your loved one can conserve his energy by doing things in an easier manner. Helping your loved one continue to do as much as possible (if he or she wishes to do so) can dramatically improve his quality of life.

 

  Services We Provide

Prime One Hospice Inc. covers all prescription drugs, over-the-counter medications, medical equipment and supplies required to treat life-limiting illnesses. In addition, Prime Hospice Care provides four types of care:

Routine Home Care: Prime One Hospice Inc. provides residential care in patients' homes and has established partnerships with long-term care facilities and nursing homes, assisted living facilities and residential care facilities for the elderly.

Continuous Care: If a patient develops the need for more constant care, we can provide shifts of hospice staff at the home for a brief period so the patient can avoid being transferred to a hospital.

Dedicated Inpatient Units: If a patient needs care beyond what can be provided at home, we have special arrangements with local hospitals and other facilities where patients may receive necessary and appropriate care and return home as soon as possible.

Respite Care: Limited to five consecutive days, respite care provides a brief “respite” for the patient’s primary caregiver by admitting the home care patient to an institutional setting without meeting the “inpatient” pain and symptom management criteria.

In addition, Prime One Hospice Inc.'s comprehensive package of services, coordinated with the patient’s attending physician, includes:

Drugs, Equipment and Supplies: Prime One Hospice Inc. covers all medications, medical equipment and supplies required for comfort and safety to treat the patient’s life-limiting illness.

A 24/7 Prime One Hospice Inc. nurse is just a phone call away. Our staff will also come to the home any time that is necessary to respond to crises related to the illness.

Bereavement: Prime One Hospice Inc. provides effective support to meet the needs of family members and friends after their loved one has died, helping them to get through the experience of loss and bereavement.
